The NHS and its partners in North West London want to know more about the care and support delivered by the community-based specialist palliative care services (or End of Life care) for adults (18+).
They want to know what is important to you and what needs to improve to ensure they deliver high quality care and excellent patient and family/ carer experience through these services.
